- Public Works & Government 鈥 Service Canada
If you have one of the following forms follow the specific instructions:
- Service Canada- CPP- Declaration of Attendance at School or University
- District/Authority Scholarship Voucher or BC Achievement Scholarship Voucher
- Email credentials@camosun.ca if you have either a;
- CATA Certification Examination Application Graduation Eligibility (CATA) form聽or
- a BACK BC Association of Kinesiologists (BACK) form.
Request a third-party form verification
International students: Use the聽.
- Complete the聽Camosun聽Third-Party Verification of Enrolment form听(笔顿贵).
- Complete the required fields on the other organization鈥檚 form.
- Email both of the above forms to聽verificationofenrolment@camosun.ca.
- Payment instructions will be emailed to you within three (3) business days.
- Make payment on聽聽($10 per form; non-refundable).
- View your receipt on聽.
- Requests will not be processed until payment is received.
- Check your email and junk mail during the聽seven (7) business day processing period.
- The verified form will be sent to your email address on file.
- No pick-up option is available.

How much does it cost to verify a third-party form?
The non-refundable fee for Third-Party Verification is $10.
How will I know if my payment has been received?
Verify your payment by viewing the receipt on聽. Requests will not be processed until payment is received.
- Financial Information > Student Finance > Account Summary > Other
- Expand All聽to view Payments and Receipt Number.
If you do not see a receipt number, then your payment has not been received. Requests with incomplete payment will be cancelled after 3 weeks.
How long does it take to receive a verified third-party form?
Your verified third-party form will be sent to your email address within seven (7) business days of payment.
Why have I not received my verified third-party form?
Verification will not be provided if:
- The $10 payment is not received.
- Your registration deposit for the requested term(s) has not been paid.
- You are not registered in any course(s) for the current or requested term(s).
- You have overdue tuition or fees owed to the college.
